OBRA-Eligible Day Services
Indiana MENTOR provides day services for individuals with intellectual and developmental disabilities (IDD) residing in long-term care facilities, such as nursing homes. Our services include individual and group rehabilitation, skill-building activities, community outings and more, both in the individual’s long-term care facility and in our dedicated center-based day programs.
Our staff provides personalized support and individualized training to boost individuals’ confidence, help them learn new skills, and enrich their lives. In addition, we provide opportunities for the individuals we serve to participate in group activities and excursions in which they can interact with peers and the community. Depending on their Individualized Service Plan, individuals may take part in goal-driven classes, activities, and rehabilitation services designed to help them reach their personal goals, including:
- Learning life skills, such as reading or using computers
- Improving physical fitness
- Strengthening communication and social skills
