As a part of Sevita (previously The MENTOR Network), Indiana MENTOR has access to the nation’s best thinking in health and human services and a successful track record of serving adults and children in home- and community-based settings.
History & Vision
During the past two decades, we have grown from a small residential program with just a few group homes to a provider of a full range of home- and community-based services for children and adults.
In 1985, when many states were moving from institutionalization to community-based care, Indiana MENTOR opened its first group home at the request of the state. In the decades to follow, our programs have generated successful outcomes for thousands of individuals and continued to grow each year. Today our group homes serve individuals with developmental disabilities, including those with extensive medical needs.
In 1992 we began offering Supported Living, which supports adults in their own homes, ranging from just a few hours a month to intensive individual supports.
In 1997 Indiana MENTOR began a therapeutic foster care program for youth at risk. In this program, young people live with Mentor foster parents who provide individualized treatment and support to help youth with complex challenges lead healthy, stable lives in the communities that they call home.
In 2008 we introduced Adult Foster Care (AFC) services. Our AFC program, also known as host home, offers senior citizens an alternative to nursing home placement. Senior citizens live with caregivers, also called Mentors, who provide assistance with daily living activities and other necessary supports so each individual can remain connected to their community.
In 2010 we started Community Based Day Program Services. Our Direct Support Professionals work with each program participant individually to plan daily activities which emphasize skill building based upon his or her needs and interests.
